Ehlers–Danlos syndromes (EDS) are a heterogeneous group of hereditary connective tissue diseases characterized by joint hypermobility, skin hyperelasticity and connective tissue fragility. They are due to defects in the biosynthesis and/or structure of proteins in the extracellular matrix. Inheritance is usually autosomal dominant.

A rotational element in knee instability with tears of the anterior cruciate ligament (ACL) is a long-established fact, and if stand-alone extra-articular lateral tenodesis has proved unreliable for long-term preservation of menisco-cartilaginous tissue at the knee, stand-alone single- or double-bundle ACL reconstruction has also demonstrated its limitations in maintaining rotational control in knee instability.

Following my recent appointment to the Bordeaux-Mérignac sports clinic, I welcomed the opportunity offered by Maîtrise Orthopédique and the SFA to visit Dr Hiroyuki Sugaya’s shoulder surgery department at Tokyo-Funabashi hospital. The main objective of my visit was to learn from Dr Sugaya’s experience in shoulder surgery, particularly as regards instability.
